Southern Berkshire Ambulance Squad is committed to protecting your health information. Our full Notice of Privacy Practices describes how we may use and disclose your medical information and explains your rights as a patient.

## Billing questions

For questions about your invoice or insurance claim, call [(413) 528-3632](tel:+14135283632) and select option 3 for billing. You may also find answers to common billing questions in our [FAQ](https://southernberkshireambulance.com/faq). 

## Request medical records

To request a copy of your Prehospital Care Report (PCR) or other medical records, contact our Privacy Officer in writing at:

Or [contact us online](https://southernberkshireambulance.com/contact). Please include your name, date of service, and a description of the records requested. We may charge a fee for copying and mailing.
